You manage to get yourself pinned, you're pretty much boned unless you can withstand 10 (TEN!) turns of merciless beating. Wow, I once played a Half-giant in the Dark Sun setting (in GURPS) who was a scholar and had "only" 16 STR, still his grapples were impossible to escape and yet we have not noticed the 10 second rule at the time... So all you can do is hope one of your allies is close and can attack the grappler. Not unrealistic, but not fair either. Absolutely. It takes a lot less force than you might imagine to inflict serious harm to a person. We're actually pretty damn fragile. I don't believe I have ever heard on the news, "X got stabbed once and died." often its more than 10 stabs and one of those happened to be lethal, or died of blood loss later. If we were so vulnerable to stabs and cuts, felines would be the dominant species on earth.

Thankfully there are a lot of points where you can break this kind of attack cycle - attacker must successfully grapple, you must fail to defend, you must fail to break free, attacker must successfully takedown, you must fail to break free, attacker must successfully pin, you must fail to break free ... then you're boned. Still, characters specifically built for grappling can be brutal, especially if they have a high enough ST (or extra arms! This is GURPS, after all) to stack the contests in their favor. Absolutely. I wasn't implying that if you jam a letter opener perfectly into someone's heart in the real world it won't be fatal. I was thinking more game-wise, with all the chaos and variables that go into 1 second of combat, it doesn't break my suspension of disbelief to think that a little dude with a little knife is a lesser threat (unless he's custom-built with some/all of the options above to specifically be "little stabby killy guy").

It all depends. There's a lot of vital organs that are a lot less protected than the heart. That one shot may not kill you instantly, but it will be fatal if the right organ is hit. Especially in an era that doesn't have access to our medical technology.
A hit to the bowels, intestine, kidneys.... All VERY doable with a knife, and without some sort of assistance, they're gonna be fatal.
There are major arteries in the upper legs that will cause you to bleed out in minutes.
However, to be clear, I'm not saying that it should be easy... Just that it is a lot easier than some might think.
