Everything goes into my backpack, unless I need maps, minis, or the props are too large to fit.
Laptop iPad Bluetooth Speaker Dice Fate Tokens / Bennies / Coins Name Tents Character Packets Maps / Minis (If I expect to need them) Props as necessary
Mostly this is a result of running games at conventions where I have to transport everything.

jazzisblues isn't lying, I've seen his backpack. That said, I've gone from bringing only character sheets, pencils, dice, and paper, to a similarly equipped backpack, and I've found that the backpack approach is superior. My backpack contains: 2-in-1 laptop (folds over and can be used as a tablet, it's really effing cool) Box of dice (grouped into little cubbies by dice set) Laptop accessories Writing implements Notepad/book ...I should note that for everything I play except D&D 5E I have PDFs of the rulebooks and character sheets so that I can tent my laptop and have rulebooks and sheets on an effective, swipe-able, touchscreen table. If I'm playing D&D 5E, I imagine I'll toss whatever books I need into the backpack as well (PHB for playing, PHB/DMG/MM for GMing, etc.) I should note that I've not played a 5E game in person - only on Hangout/Roll20 - so I haven't actually tested that.

When I'm a travelling GM, I've got my Box: Books, character sheets, my notes, iPad (OneNote convert here), miniatures I think will be needed for the session, dice, tokens, markers. The host has a setup for music and also a widescreen TV hooked to his laptop: I send him files that will have pictures (specific monsters/enemies) I can have pop up on the screen so people can see what they're facing.
When I'm home, it's the same Box, but with more minis and terrain at my disposal.

Back in the day, I used to carry all the books for the system I had - up to three good-sized totes. Eventually, I started running off of a laptop with OneNote and pdfs. So now it's the laptop, dice, scratch pad and pencils.

I replaced it with a Surface 2 Good call, I love the Surfaces. I had an original Surface RT for a long time - which I used for gaming - but I sold it to get my current laptop (the aforementioned Toshiba Satellite Radius 2-in-1). A bunch of executives at my work use Surface Pro 3s, those are really choice machines. And Microsoft just announced the Surface 3, which fills the niche the RT devices used to fill, and has really caught my interest. For the longest time I resisted electronics at the table ("YOU CAN HAVE MY PRINTED CHARACTER SHEET WHEN YOU CAN PRY IT FROM MY COLD, DEAD HANDS") but I've come around and accepted tablets/laptops as valuable gaming accessories.

I like PDF character sheets a lot-- once they're filled out and complete. I hate filling out PDF forms. Therefore, I really like paper character sheets for the first session. For every session after that, electronic all the way yay!!

When I was running a game, which I'm now not doing (for reasons) I would bring my laptop, my dice and the two core books (we were playing Dresden Files). Everything else was either electronic on and my laptop or accessible via Uncle Google. I'll be a laptop GM from now on, make my life easier.
EDIT: Index cards! For the love of God, don't forget the Index Cards.

Index cards are practically required for FATE based games. I started using 3x5 pieces of plastic card and wet erase markers. I had different cards for different types of aspects so they could tell at a glance what aspects were on the environment, NPCs, this zone, etc.
