I've been reading Never Unprepared at work to try to get better as a GM. One of the key points that Phil Vecchione stresses is to take the time to brainstorm. I was between calls at work, so I decided to do just that. In the spirit of Jackercon, I decided to try to come up for a Cthulhu scenario. Here's how the brainstorming went:
(transcribed from my notepad) Cult and Conspiracy Can't be seen Octopus intelligent (added after)->Atlantean Cats? -> Egyptian Artifact
Atlantians are Octopodidae creatures that worship Cthulhu and want to bring him to this realm. They get around by using humans as vessel a la Invasion of the Body Snatchers and the Futurama slugs. I thinking b/c of the beak they go in through the nape of the neck and control the bodies from the head. The ultimate goal, of course, is to bring Cthulhu to this realm. The timing being just right on 12/23/2012. The location of the portal being Apocalypse Island, which lines up really nicely with planets, thus giving the sufficient amount of cosmic energy to open the portal.
Unfortunately that's as far as I got. If anyone wants to pick this up and run with it, be my guest. I'll add more if I can come up with a reason as to why the PC's want to get involved. That's where I am stuck right now.

A quick idea for why the PCs might be involved: The PCs are investigators / private eye types who were unknowingly hired by agents of the Octopodidae to track down some missing antiques (ie ritual objects). The PCs track them down to members of the Cult of Sekhmet, disrupt a minor cult meeting and hopefully recover the objects. After reporting back to their client the remaining Cult members are then murdered by the Octopodidae and the PCs framed for it. Cue trying to work out who framed them while on the run from the police, uncovering the truth about the Octopodidae and then trying to stop the end of the world.

Maybe the PCs are on a plane or boat, heading home for Christmas, when it's diverted to the island by the captain and his octopus-brain-hitchhiker?
Although, in a modern scenario like this, I'd consider making it a "hiding in plain sight" thing. I can totally see some group trying to come up with their own Burning Man kind of thing on a remote island, only to get enough bodies in one place to run a ritual.

Depends on how much ahead-of-time you want to start the thing. I'd maybe have the PCs get involved in a car accident - like, they're on a bus that hits a car, or something along those lines. When at least one of them jumps out to help (there's always at least one), the driver has had a case of severe head trauma. Roll perception! Ew! You see this... octopus... thing slide out of the wound!
And have it jump down a storm drain.
So now they know there's these things, but short of cracking open someone's skull they don't know for sure. And they might find some clues in the car, maybe...

1. The players are hired by a woman who is convinced her husband is having an affair. He has grown secretive and disappears for hours at a time in the evening. (Or the woman is a friend, just asking for help.) While following him, they watch him duck into a meeting with unfamiliar people. That's when the Octopods converse without pretense. When they go back to her to warn her, she is acting strangely and unmoved by the revelation. Something got her too.
2. The players have won a day trip to Apocalypse Island! Lucky them, good thing they filled out that card at the Tastee Freeze. Wait, why aren't they being allowed to leave the island? Hey wait, where are you taking us? What the...

Any good ritual needs a ritual sacrifice. Any good sacrifice needs a specific blood lineage. Any good lineage has (hundreds of years ago) been instrumental in sealing Cthulhu's portal. Any good sealing ritual has instructions hidden within a family heirloom. Any good recipiant of said heirloom has a run in with the opposition trying to prevent said family member from discerning the secret of the heirloom. Said event triggers interest in heirloom, heirloom leads to ritual sight, ritual sight is teaming with cultists, cultists are preparing for the return of Cthulhu. .....uhh where was I going with that?
