I will say this. Horror games are my favorite. Done well, they can affect a player enough that they will talk about it forever.
If you want to make them paranoid, some previous suggestions work well.
Dont ask for rolls and perception checks. Just add things they notice from time to time.
The water has a copper taste to it. A light flickers, dims, goes out, or flares slightly. It gets warm or cold. They catch movment in the corner of their eye. But at the early stages, you give them a plausible explanation.
The water is stale or is running off through rocks. A breeze rolls in. There was cold or warmth due to a natural feature. The shadow was a cloak in the wind.
Then take out the plausible. Somethinf flickers, but there is no wind. A shadow was there... but it is a bright day and there is a wide open space.
Take the normal and twist it. I once told a character they felt the presence of eyes on them. And when they looked around, they saw a cat staring at them. And then another. And as they turned to take it all in, a hundred cats were sitting still, staring at them, following them with their eyes. Have them walk past a house where they hear singing, but nothing is there. They feel a brush on their shoulder as if someone was wiping off dust. They see footprints that disappear in the middle of a room.
Then twist things further. The healing potion tastes like blood, and as they drink it, they feel their teeth shift. As a friend crits a bad guy, the friends face shifts for a moment, looking demonic. They hear whispers at night when they are alone, or their friends sleep.
Be evocative with description, detail the minutae, but paint evil in broad strokes.
They are crawling down a tunnel, and gobs of worms and maggots fall upon them. The air stinks of decay, and a sludge forms at their feet as they trudge through the water, dirt and bones in the tunnel.
But when they come to the end, it is "larger than you comprehend at first. A mass of flesh and bone, but things dont sit where they should. Many of it's mouth start to open and drool." Just a few cents.

I personally love FS2 and 7th Sea, and I find both do well, so long as the GM is pretty much into it. And I am super excited about the new 7th Sea, as the char sheet posted seems to limit the skills down - I get the feeling it might be faster.
I have used the Dynamic D6m (Fireborn) hacked for some swashbuckling stuff, and that was fun.
Also, I think John Wick posted some dueling rules to one of his Wicked Words that looked pretty good.
Savage Worlds can do it somewhat. There is a book on unusual fight locations that give you ideas how to do it.
I have also done decently with FATE.
I love dynamic fights that move around. So, I try to put it in all my games.
Some games that look like they might be fun to try, though I have not yet, are Spellbound Kingdoms, All for One, and Dragon Brigade.

L5R. I play a Scorpion Shugenja, a Jade Magistrate, and my Phoenix Yojimbo, our Unicorn Courtier, and his Crane Yojimbo are currently stuck behind the wall due to foul deeds of mass murder in Crab Lands.

I second Cypher, and throw in Feng Shui (And the new one is Fantastic).
Fate and Savage worlds are both fantastic, but I sort of miss tossing people fate chips/bennies in a physical sense. lol

Well, I jusy got Feng Shui 2 in the mail, I picked up the Infinity Beta, but most recent purchase was probably the Savage Wolds Solomon Kane book. Had been planning on that one for a bit.

I would be okay with it.
Backlash has the potential to do no wounds, or multiple wounds, or even kill if something goes severely wrong.
Having a backlash of one wound is serious, but it is still balanced. It's almost like the spirits saying they are taking a "standard price" which fits for some theories on spirit magic.
I personally would let them use their bennies to still try and soak. Pass it off as a "Well, you apologized, so... I'll let you off... this time."
Also, if the players are aware of the risk and consequence, then it is still the same basic informed choice as the rolled damage. More so, actaully, since they have an exact number. Less of a gamble, and more of a direct trade.

I managed to get in on the kickstarter near the end (and spent entirely too much on it), and love what I read so far. I am unsure if my current group will be willing to try it, but I hope so. I have to admit, compels are a grea idea. I think I could have a lot of fun with those.
